Досрочная сдача промежуточной аттестации
Прием заявления от обучающихся СКУ им. М. Козыбаева при наличии документа, подтверждающего уважительную причину необходимости досрочной сдачи промежуточной аттестации (справка с медицинского учреждения, командировочный лист, письмо на Председателя Правления - Ректора об участии обучающегося в соревнованиях и т.д.).
Срок оказания услуги: 2 (два) рабочих дня.
Перечень документов:
1) заявление на имя декана/директора факультета;
2) удостоверение личности услугополучателя (либо его законного представителя) или студенческий билет (оригинал требуется для идентификации);
3) копия квитанции об оплате за обучение (обучающимся на платной основе);
4) документы, подтверждающие уважительную причину досрочной сдачи промежуточной аттестации (справка с медицинского учереждения, командировочный лист, письмо на имя Ректора Университета об участии обучающегося в соревнованиях и т.д.).
