Продление сроков промежуточной аттестации обучающимся
Услуга «Продление сроков промежуточной аттестации обучающимся» - услуга СКУ им. М. Козыбаева, которая оказывается обучающимся СКУ им. М. Козыбаева при наличии документа, подтверждающего уважительную причину продления сроков промежуточной аттестации (справка с медицинского учреждения, командировочный лист, справка с места работы, приказы, распоряжения, письмо на имя Председателя Правления - Ректора об участии обучающегося в соревнованиях и т.д.).
Срок оказания услуги: 2 (два) рабочих дня.
Перечень документов:
1) заявление на имя декана/директора факультета;
2) удостоверение личности услугополучателя (либо его законного представителя) или студенческий билет (оригинал требуется для идентификации);
3) документы, подтверждающие уважительную причину продлениия сроков промежуточной аттестации обучающимся (справка с медицинского учреждения, командировочный лист, письмо на имя Председателя Правления - Ректора об участии обучающегося в соревнованиях и т.д.).
